This Privacy Policy explains how Rocklin Healthcare (“we,” “us,” or “our”) collects, uses, and shares information when you visit rocklinhealthcare.com (the “Site”), call us, or contact us through the Site. Medical services are provided by 5 Rivers Health Inc, 6839 Five Star Blvd, Suite C, Rocklin, CA 95677.

Not for emergencies

The Site, its forms, and our phone line are not monitored for emergencies. If you think you are having a medical emergency, call 911 or go to the nearest emergency room.

Health information and HIPAA

When you become a patient, health information we create or receive about you is handled as required by the Health Insurance Portability and Accountability Act (HIPAA) and California law, including the Confidentiality of Medical Information Act. How we use and disclose your protected health information is described in our Notice of Privacy Practices, which is provided separately to patients and is available from the clinic on request.

Website forms are for scheduling and general questions only. Please do not send medical records, detailed symptoms or clinical history, Social Security numbers, or insurance member IDs through the Site. Share those details with your care team by phone or in person.

Information we collect

• Information you give us. If you use the contact or membership inquiry form, we collect what you enter, such as your name, phone number, preferred contact time, the service or membership you are interested in, a brief non-clinical reason for your request, and, if you choose, your insurance carrier name.
• Phone calls and messages. If you call or text us, we receive your phone number and the content you share with us.
• Information collected automatically. Like most websites, our hosting provider and any analytics tools we use may collect technical data such as IP address, browser and device type, pages viewed, referring page, and the date and time of your visit.

Cookies and analytics

The Site may use cookies, pixels, and similar technologies to keep the Site working, understand how visitors use it, and measure the performance of our advertising. You can block or delete cookies in your browser settings; some features may not work as well if you do. We do not use these tools to collect information you enter about your health, and we configure them not to send form contents to advertising platforms. Online booking (Tebra)

Online appointment booking on the Site is provided by Tebra, a third-party scheduling service. When you use the booking tool, the information you enter is collected by Tebra on our behalf and is subject to Tebra’s own terms and privacy policy, as well as our obligations as your healthcare provider.

How we use information

• To respond to your request, schedule appointments, and explain our services and memberships;
• To call or text you about your request, if you have asked us to;
• To operate, secure, maintain, and improve the Site;
• To measure and improve our advertising and outreach;
• To comply with law and protect our rights, patients, and staff.

How we share information

We do not sell your personal information. We share information only with service providers that help us run the clinic and the Site (such as website hosting, scheduling, phone and messaging, and customer-relationship tools), when required by law or to protect safety, or as part of a business transfer such as a merger or sale. Service providers may use information only to perform services for us. This sharing excludes text messaging originator opt-in data and consent.

Phone and text message consent

If you give us your phone number and request contact, you agree that Rocklin Healthcare may call or text you at that number about your request and appointments, including by automated means. Consent is not a condition of receiving care or making a purchase. Message frequency varies. Message and data rates may apply. Reply STOP to opt out of texts at any time, or HELP for help. Carriers are not liable for delayed or undelivered messages.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing to subcontractors in support of services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Telehealth

If we offer you a telehealth visit, it is conducted through a secure platform, and the health information exchanged is protected as described in our Notice of Privacy Practices.

Your California privacy rights (CCPA/CPRA)

If you are a California resident, you may have the right to: know what personal information we collect, use, and disclose; access a copy of it; correct inaccurate information; request deletion; and opt out of the “sale” or “sharing” of personal information (including for cross-context behavioral advertising). You may also limit our use of sensitive personal information where the law provides that right. We will not discriminate against you for exercising these rights.

Medical information governed by HIPAA or the California Confidentiality of Medical Information Act is handled under those laws and the Notice of Privacy Practices rather than this section.
