Medical services are provided by 5 Rivers Health Inc, 6839 Five Star Blvd, Suite C, Rocklin, CA 95677. By using the Site, you agree to these Terms. If you do not agree, please do not use the Site.

Emergencies

Do not use the Site for medical emergencies. If you think you have a medical emergency, call 911 or go to the nearest emergency room.

Not medical advice

Content on the Site is general information only. It is not medical advice, diagnosis, or treatment, and it is not a substitute for an evaluation by a qualified clinician. Using the Site, submitting a form, or calling us does not by itself create a physician–patient relationship. Whether a service, membership, or therapy is right for you is decided by a licensed clinician after an evaluation.

No guarantee of results

Individual results vary. Nothing on the Site guarantees any specific clinical outcome. Services and memberships described on the Site may change and are offered only where clinically appropriate.

Telehealth

If telehealth visits are offered, they are available only to patients located in states where our clinicians are licensed and only when clinically appropriate.

Appointments and third-party services

Online booking is provided by Tebra, a third-party service with its own terms and privacy policy. The Site may link to other third-party websites, such as the NPI Registry. We are not responsible for third-party sites or services.

Phone and text messages

Rocklin Healthcare Text Messages. If you opt in, Rocklin Healthcare (medical services provided by 5 Rivers Health Inc) will text you appointment confirmations, reminders, scheduling updates and callback follow-ups. Message frequency varies. Message and data rates may apply. Reply STOP to opt out, HELP for help. Support: (916) 964-2775. Carriers are not liable for delayed or undelivered messages. Texts never contain diagnoses, results or other health details. See our Privacy Policy and these Terms.

Acceptable use

You agree not to misuse the Site, including by interfering with its operation, attempting unauthorized access, submitting false information, or sending detailed health information through website forms.

Intellectual property

The Site and its content, including text, graphics, logos, photos, and video, are owned by or licensed to Rocklin Healthcare and are protected by law. You may view and print pages for personal, non-commercial use only.

Disclaimer of warranties

The Site is provided “as is” and “as available.” To the fullest extent permitted by law, we disclaim all warranties, express or implied, including warranties of accuracy, merchantability, fitness for a particular purpose, and non-infringement. Limitation of liability

To the fullest extent permitted by law, Rocklin Healthcare, 5 Rivers Health Inc, and their owners, clinicians, staff, and service providers will not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or relating to your use of, or inability to use, the Site. Our total liability for any claim relating to the Site will not exceed one hundred U.S. dollars ($100). This section does not limit liability for professional medical services, which are governed by applicable law, or any liability that cannot be limited by law.

Governing law

These Terms are governed by the laws of the State of California, without regard to its conflict-of-laws rules. Any dispute relating to the Site will be brought in the state or federal courts located in Placer County, California, unless the law requires otherwise.
